Outreach Care Coordinator

Case Coordinators will work alongside clients to navigate a network of support on behalf of Opportunity House’s outreach programs and services.

PRIMARY RESPONSIBILITIES

  • Conduct regular street outreach to identify and connect with individuals experiencing homelessness.

  • Create consistent relationships to understand the unique needs and challenges of SO Clients.

  • Provide information about available resources, including shelters, food services, healthcare facilities, and social services.

  • Ensure individuals understand their rights and the voluntary nature of their participation.

  • Maintain and document all interactions with clients in order to monitor progress in HMIS.

  • Enroll and support clients with ECM and CS.

  • Assist in triaging clients to the appropriate resources and services.

  • Complete reports to track outreach outcomes and trends, contributing to program evaluation and improvement.

  • Attend various meetings as a representative in the JPA, CoC, CES, HMIS, and Homeless Roundtable committees.

  • Collaborate with community partners, local agencies, and service providers to enhance outreach efforts and expand resources.

  • Commit to serve our clients with excellence in light of our mission and values.

Rate of Pay:
$22-25/hr, DOE

  • Basic computer knowledge, and a willingness to learn new programs (as needed)

  • Sensitivity to cultural diversity and a commitment to promoting inclusivity

  • Ability to lift 25lbs+ and sit for long periods of time

QUALIFICATIONS

Required:

  • High school diploma or equivalent,

  • Previous experience in street outreach, social services, or community organizing

Desired:

  • Lived experience and peer support specialist certification

  • Bachelor’s Degree and 1+ years of Human Services experience
