Opportunity House Shelter Case Manager
The role of a Shelter Case Manager encompasses assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s and family’s needs. This position will work to address the physical, psychological, and social needs of clients, and help them achieve self-sufficiency.
PRIMARY RESPONSIBILITIES
Provide personalized one-on-one support to clients
Make referrals to community resources and assure linkage with those resources
Assist clients in identifying and achieving their personal goals
Monitor, evaluate, document, and track the progress of clients, and revise case plans as needed
Offer educational and vocational services to clients
Establish a system for finance management
Support clients in their recovery from substance abuse and other forms of trauma
Help manage client crises
Assist with the building of community living and life skills programming
Collaborate closely with external agencies, schools, and community partners to enhance support networks

Sensitivity to cultural diversity and a commitment to promoting inclusivity
Ability to lift 25lbs+ and sit for long periods of time
QUALIFICATIONS
Desired:
High school diploma and 3+ years of Case Management experience, OR
Bachelor’s Degree and 1+ years of Case Management experience
